Instructions to get your Boxer Passbook

To complete your registration and  receive your Boxer Passbook:

1. Upload the following into your USA Boxing webpoint profile:

* Headshot Photo (selfie) - no need for 2 passport-sized photos

*Birth Certificate or Passport

*Physical (only upload the Clear to Participate page, signed & dated by Healthcare provider)

2. Email billwellsutsa@yahoo.com & include:

*Boxer’s Name

* Mailing Address, City, Zip

* If Boxer is under 18 years – include parent’s name for mailing purposes

* USA Boxing Member ID Number
