Renewing Athlete Details

If you have your passbook from a previous year, you do not need to send anything in to

your registration chair.

You MUST print your Membership Card and tape it to the first page of your

passbook.

Your Membership Card is your only proof of registration and without it you will

not be able to box!!

If you DO NOT have your passbook, you must contact your registration chair to get a

replacement book. There may be a fee charged, not to exceed $50.

Physical Exam: All athlete members of USA Boxing must get a yearly sports physical

and for the health of your community, make sure your vaccinations are up to date. We

have created a form specifically for this purpose.
